1. Visit the official Surat Municipal Corporation website.

5. Enter your seat number (bench number) and date of birth.

1. Download the “Query Reporting Form for Provisional Answer Key” issued along with the notice.
2. Fill in all required details in the form, including candidate seat number, post name, question paper set (A/B/C/D), question number, the question, the options, the key uploaded on the website, and the key suggested by the candidate, along with a remark or reference supporting the suggested answer.
3. Attach a valid, colour scanned, self-signed copy of documentary evidence supporting the objection.
4. Attach a copy of the exam call letter and a photo ID proof (Aadhaar Card, Voter’s ID, Driving License, etc.).
5. Scan the completed form along with the supporting documents.
